Together We Serve & Together We Make a Difference

About

Who we are..

Why a Co-op?

A co-op is "an autonomous association of persons united voluntarily to meet their common economic, social, and cultural needs and aspirations through a jointly-owned and democratically-controlled enterprise.”

AZFC uses a model where all parties are equal and working cooperatively to meet a higher level of service which includes a strong philosophy of supporting the goals and desires of the identified person using person-centered planning and goal setting.

Why Become A “Host” Family With AZFC?

We Do One Thing & We Do it The Best!

  • No employees, only owners partnering with families to provide host (aka developmental) home services.

  • AZFC minimizes overhead expenses to provide a maximum reimbursement rate to host families.

  • We are looking for families who share the same service philosophy of supporting individuals who want to live a self-determined life.

  • Our caseloads are small for better management, consistency in approach, support and communication flow.

  • We focus on person-centered approaches which contrasts the typical adult developmental homes that have the provider driving the service.

  • We use newer technologies to streamline and improve the data collection process as well as training opportunities for families. All data collection is gathered in real time using cloud-based services to gather and store all the data.

Why create a new company?

It's time to focus on quality services

  • Over the past few years, many large organizations have transitioned from serving people with I/DD to diversifying into other areas and states. At AZFC, we believe it is important to focus on a single service to ensure that people have living options in the least restrictive and most normalizing manner.

  • AZ Families Co-op is an organization with a singular focus on Host Homes. AZFC’s business model minimizes overhead, offers and uses the latest technology, and provides the highest reimbursement rates for families.

  • We use the word "Co-op" because we believe a partnership exists between AZFC, DDD, Host Homes, people receiving services, and their families. We maintain the mindset that everyone succeeds when we work in a cooperative environment!

What We do....

We call our homes "Host Homes" to emphasize the idea that people are invited into the home with the expectation of having the same rights and responsibilities as other adults in the home.

  • AZ Families Co-op, LLC (AZFC) provides individualized supports to host families and individuals residing in host homes (aka developmental homes). Only owners provide the direct services to recruit, license and monitor our homes. We believe this direct approach will allow for greater oversight and minimal turn-over for consistency.

  • The owners provide guidance and support to ensure that individuals who reside in the homes are able to meet their goals and desires. We are committed to managing administrative costs so that the maximum revenue is utilized directly to enhance the lives of the people who reside in these homes.

  • Traditionally, developmental home providers have been the principal decision-makers for their households. We believe that all stakeholders benefit from a more inclusive service model that affords options based on the individual's stated goals. Additionally, because the owners directly benefit from their own work, there is a stronger focus on attaining satisfaction from all stakeholders, including the individual, guardian/representative, Division staff, and the host home providers.

  • If stakeholders are unhappy with services, the owners have a direct consequence as opposed to traditional models, which use employees who are compensated based on work hours rather than results.

  • Arizona Families Co-op LLC (AZFC) recruits, licenses, and monitors independent contractor “host” families through a contract with the Department of Economic Security/Division of Developmental Disabilities.

  • AZFC recruits host families of varying backgrounds to provide the best potential match for individuals seeking living arrangements that offer a safe environment that is responsive to the goals and desires of the identified person. A strong philosophy of self-determination for all parties, including the principles of accountability, responsibility, and advocacy, will be the standard.
