Requirements: (not all inclusive)
AZFC does not accept all applications and has high expectations for host families in meeting our service philosophy.
Must be 21 years of age or older
Must have a desire to support real changes in the life of the individual living in your home
You can be married or single, divorced, partnered, or cohabiting
Pass a medical evaluation
3 Character references (not related to you)
Must pass a Criminal background check (CPS/APS records check)
Home study (family assessment)
Pre-service education: 25 hours or more of classroom training
Have sufficient Income to meet your needs
Not have employment that conflicts with the care and supervision of adults placed by DDD
All persons in the home 18 years of age or older must be fingerprinted
Pass a Life-Safety home inspection by DES/OLCR
Provide private sleeping arrangements
Provide transportation
Complete all required training

**10 hours of training is required yearly to renew the ADH license
How long is the process?
Getting licensed can take up to 6 months requiring a strong commitment to meeting timelines that include training, interviewing and environmental planning.
These requirements are the responsibility of both you and AZFC.
